Yes that would be the correct shim, the smaller number listed is the seatpost size and the larger number is the frame size.

It doesn't look like we carry a shim that would allow a 25.0mm or 25.4mm seatpost to work in a 25.8mm frame. The smallest shim we carry will adapt a 25.4mm post in a 26.2mm frame.

While it isn't ideal, it should work. The biggest issue would be getting even torque across both shims and the seatpost.

Cane Creek makes a 27.2 to 30.04 but not a 25.4 to 30.04, you might be able to stack the 25.4 to 27.2 and the 27.2 to 30.4 shims.
